† Grades K - 12
1. Application: one application per student with a non-refundable fee of $50 each.
2. Family interview: When there is an available space, the school registrar will contact you to schedule anappointment with the principal or vice principal. This is your opportunity to discuss Calvary’s educational philosophy and ask any questions you may have regarding our policies and programs.
3.Contract:Sign Parent/School Financial Agreement.

† Grades 6-12
In addition to the items above, middle and high school applicants must also provide the following before the family interview is scheduled:
1. Proof of Hepatitis B and second MMR vaccinations for students entering the 7th grade.

† Kindergarten
In addition to the items for grades K-8 above, kindergarten applicants must also provide the following (these must be on file at Calvary before the family interview is scheduled):
1. Immunization verification from the child's doctor.
2. Birth certificate.
3. Kindergarten Readiness Test: Required for children with 5th birthdays between July 1 and December 1.
